- Used for recording prescriptions dispensed to patients at the Receiving House Royal Park Mental Hospital 1941 - 1956, a mental health hospital in Victoria, Australia.Entries handwritten in ink. Gives names of patients, dates and contents of prescriptions. In front is section for staff to sign for pharmacy keys. At back is list of bulk orders for each year and inside back cover prescription for routine "build up" of patients.

- It is a large, lined notebook with blue oilcloth cover, halfbound in blue leather. End-papers grey.
